Role: Clinical Pharmacist Assignment: Humana Pharmacy Location: San Antonio, TX

Assignment Capsule

In this role you will directly interact with our members, physicians, physician office staff and Humana care management teams to help drive safe and optimal medication use. This provides the opportunity for you to practice at the top of your license to positively impact quality of care and overall health care spend.

• Provide Medication Therapy Management (MTM) services to eligible patients (face to face and telephonically). This includes both comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs). • Partner with Humana and the provider to assist in quality measure gap closures and pharmacy cognitive services. • Support medication reconciliation initiatives to ensure appropriate drug therapy and reduce inpatient readmission. • Identify opportunities for cost reduction that are beneficial to the member and the plan. • Educate Humana members, physicians, physician staff on their benefits/coverage and other services available from Humana. • Assist in the creation, education, monitoring and improvements of clinical initiatives. • Support collaborative practice agreements as allowed by state law with local physicians as needed. • Screen for social needs assessment (food insecurity, health literacy, mental health) and provide appropriate guidance/referral for ongoing support and resources. • Coordinate with Humana Pharmacy Clinical Program team to support outreach initiatives • Clinical education resource for Humana Pharmacy staff • Support Humana market initiatives (i.e., Bold Goals) to achieve improved member health • Provide immunization services (influenza, pneumonia, shingles, etc.) as needed • Support opioid management initiatives • Provide relief staffing support at the local Humana-owned retail pharmacies as needed

* Role Essentials*

• Bilingual; English/Spanish • Ability to handle multiple tasks; prioritize and demonstrate flexibility • Bachelor of Science in Pharmacy (BS Pharmacy) or Doctor of Pharmacy (Pharm.D.) • Clear and active pharmacist license in the state(s) in which you are required to practice • Ability to be licensed in multiple states without restrictions if needed • Proven history of success in pharmacy operations (preferably retail) • Proficiency in providing Medication Therapy Management (MTM) services including comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs) • Ability to drive to multiple physician clinic or Humana Retail Pharmacy locations (valid, unrestricted driver’s license and dependable vehicle) • Immunization administration certified or willingness to certify and train

Role Desirables

• Accredited pharmacy residency (1 year ambulatory practice) or 3 years equivalent ambulatory pharmacy practice experience • Previous health plan and or Medicare experience • Familiarity with CMS Stars quality initiatives

Additional Information

Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
