Humana is seeking a Care Coach, CMDM (RN) to telephonically assess Medicaid, members in 7 domains of functioning; evaluate member needs and requirements to achieve and/or maintain optimal wellness state. You will guide members and their families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. Work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to manage a member’s physical, environmental and psycho-social health issues.

Other responsibilities include: * Proactive telephonic outreach to eligible Humana members and engage participation in care management programs.

  • Actively participate in all enrollment activities.

  • Complete physical, psychological, emotional and environmental assessments, for the purpose of providing appropriate, timely interventions to ensure provision of optimal care.

  • Coordinate community care and services as deemed appropriate.

  • Works collaboratively with other members of the Interdisciplinary team-to include; Humana Care Managers-Social Services, Field Care Managers and Community Health Educators.

  • Understand clinical program design, implementation, and management, monitoring and reporting.

  • Comply with performance and reporting standards as defined by Humana Corporation.

  • Additional responsibilities as deemed appropriate by Humana leadership. Assignment Capsule As a Care Coach, CMDM (RN) you will engage our members to develop lifelong well-being and health.

    Humana Medicaid is seeking Care Coach, CMDM Nurse who will telephonically assess, plan, implement and coordinate all case management activities for our members. You will educate members on making choices to assist them to achieve better overall health.

  • Work with identified members telephonically to assess their care needs.

  • Assist in planning and implementing interventions to meet those needs.

  • Assist in coordinating services, and monitoring and evaluating the case management plan against the member's personal goals.

    Role Essentials

  • Active RN license in the state of Florida

  • Ability to be licensed in multiple states without restrictions

  • 3 years clinical adult care experience

  • Ability to work independently under general instructions and with a team

    Role Desirables

  • BSN

  • Health Plan Experience

  • Prior experience with Medicare & Medicaid recipients.

  • Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems.

  • Experience with health promotion, coaching and wellness.

  • Call center or triage experience

  • Knowledge of community health and social service agencies and additional community resources.

  • Previous telephonic case management experience working in the medical/insurance field.

  • Previous managed care experience.

  • Basic knowledge of complex care management and care management principles

  • Experience with motivational interviewing

  • Experience with behavior change, health promotion, coaching and wellness.

  • Bilingual: Must be able to speak, read and write in primary and secondary languages without limitations or assistance.
