Role: Behavioral Health Specialist
Assignment: Partners in Primary Care Location: Independence and Kansas City, Missouri
Role Description: Be a part of the Clinical Care Team-- as a Health Guide, you will engage our patients to develop lifelong well-being and health. Partners in Primary Care is seeking a Behavioral Health Specialist to make a difference in people’s lives and will be responsible for working with the Care Team to recognize and address psychological symptoms and conditions using the bio-psychosocial model.
Essential Job Functions:
Provides professional expertise and consultation to the care team
Provides crisis and risk assessment to the provider and other members of the care team
Performs assessment and brief treatment for a wide range of psychological disorders using brief therapy
Assesses the treatment needs of the members
Develops care plans based on diagnostic assessment information that is documented appropriately, including identified goal(s), behavioral/psychological problem statement, measurable objectives and estimated time frames for achievement
Advises members and family members in methods that facilitate a capacity for self-care and a movement towards a healthcare partnership between the healthcare teams, the members, and their families
Apply knowledge of and experience with a broad repertoire of psychotherapeutic skills from a variety of theoretical perspectives and focused brief therapy (e.g. play therapy with children, cognitive behavioral and psychodynamic therapy with adults, psychoeducational therapy and family therapy)
Provides effective triage of members in need of specialty behavioral health
Develops educational, training materials and conducts sessions or disseminates information to care team on recent advances in the field
Present clinical presentations at clinical review meetings
Chart reviews and peer shadowing completed on a regular basis
Maintains documentation in compliance with state, federal, professional and ethical guidelines
Independently develop, coordinate and facilitate psycho-educational classes, groups and member activities as relevant to treatment plan/program

Master’s in behavioral health field including; social work or counseling
Licensed LPC or LCSW
3 years working in a clinical setting
Trained in substance abuse identification and treatment
Experience in a fast pace/high volume environment
Experience with MS Outlook
Basic Computer knowledge
Must be a team player with excellent communication skills and strong emotional intelligence
This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB
Ability and flexibility to work between locations, based on business need
Three years’ experience working in an outpatient setting ( Primary Medical Care)
Trained in behavioral medicine and health psychology concepts
Trained in social, systems and familial aspects of psychological and medical conditions
Trained in substance abuse identification and treatment
