Assignment Capsule Be a part of our Clinical Space- Provide guidance to facilitate interaction or services that connect people with lifelong well-being.

The HAH Member Engagement Associate will initiate telephonic outreach to members identified by Humana At Home as viable candidates for Care Management and engagement. You will explain Humana’s Care Management program and obtain consent for enrollment.

Other responsibilities include:

• Document per protocol and communicate pertinent information to designated person and/ or persons.

• Meet or exceed member engagement goals on a consistent basis.

• Participate in the promotion of the multi-disciplinary healthcare team.

• Work collaboratively with other members of the clinical leadership team.

• Relate to a production environment and understand process flows.

• Maintain and improve quality results by adhering to standards and guidelines; recommending improvements as identified.

• Must meet all compliance and privacy requirements set forth for proper member engagement.

• Additional responsibilities as deemed appropriate by Humana At Home Leadership.

  • Role Essentials *

    • High School Diploma or equivalent

    • Six months of recent relevant experience

    • Intermediate to advanced computer skills and knowledge of Microsoft Word and Excel

    • Able to use a variety of electronic information processing tools and multiple software programs

    • Professional, interactive written and verbal communication skills

    • High level of detail and organization skills

    • High level of knowledge retention

    • Able to multi-task without losing sight of accuracy & quality

    • Able to establish priorities, work independently and proceed with objectives without direct supervision

    • Able to thrive in a fast paced environment

    • Able to work a full time (40 hours) and flexible work schedule as early as 9:30am as late as 8:00pm to include some evenings and weekends

  • Role Desirables * o Associate’s degree or equivalent from two-year college or technical school OR six months to one year related experience and/or training

    o Related experience in healthcare, customer service and/or call center

    o Experience working with the senior adult population

    o Experience with outbound calls

    o Experience working on an automated/auto dialer system

    o Bilingual (English/Spanish); speaking, reading, writing, interpreting and explaining documents in Spanish and English

    o Previous experience with Humana at Home as a Referral Specialist, Care Concierge or Referral Intake Specialist Reporting Relationships You will report to a Front Line Leader.

    Interview Format

    As part of our hiring process for this opportunity, we will be using an exciting interviewing technology called Montage Voice to enhance our hiring and decision-making ability. Montage Voice allows us to quickly connect and gain valuable information for you pertaining to your relevant skills and experience at a time that is best for your schedule.

    If you are selected for a first round interview, you will receive an email correspondence inviting you to participate in a Montage Voice interview. In this interview, you will listen to a set of interview questions over your phone and you will provide recorded responses to each question. You should anticipate this interview to take about 15 to 30 minutes. Your recorded interview will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.

Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
